- Hiatal hernias are a very common medical condition and present as the pathologic passage of the foregut through the esophageal hiatus. While most hiatal hernias are asymptomatic, presenting symptoms can easily progress to gastroesophageal reflux disease (GERD). It is important to diagnose and treat GERD to avoid more severe esophageal conditions. Understanding the anatomy of hiatal hernias through 3D visualization analysis can aid medical students in assessing the clinical considerations of hiatal hernias, GERD, and the risk for more severe esophageal conditions.

This poster reviews the literature on the causes and consequences associated with low accessibility to mental health care, such as the shortage of mental health professionals and health insurance coverage. The consequences are significant and impacts millions due to receiving less-than-optimal or no mental health care. The solution to this issue can be seen through an increase in mental health professionals and psychiatrists nationwide. - In the study, we are analyzing the human genome in order to determine patterns that may tell something about how DNA functions. Patterns require an explanation: it is highly improbable that they are random occurrence. These patterns may hint to something about how DNA functions. There are known patterns already discovered in DNA. For example, in the coding portion, three base pairs translate to a specific amino acid. In the non coding portion, however, specific patterns are not as simple. We will search for patterns by applying a coding system that turns DNA into music. Music may serve as a powerful tool because we will be able to use to analyze long codings in a short period of time, and the entire phrases will connect in some way. When you listen to a song the order of the chords and phrases have an impact on the entire sound. DNA is the same way; the individual sequencing tie in together to replicate and transcript our genetic material. We will use known knowledge of DNA to base our system of musical coding off of, such as the individual nucleotide sequencing, amino acid coding and protein binding sites. Specific musical assignments will be given to each of the pairings. For example, an adenine base pair could be assigned a C major chord. The DNA will then be played using a computer programing software. The DNA sequences will then be heard and analyzed for specific patterns.

- Mobile Integrated Healthcare (MIH), also called Community Paramedicine (CP) or EMS 3.0, is a novel approach to prehospital care. With the use of specially trained paramedics, MIH-CP seeks to fill the current healthcare gaps in communities by facilitating an interconnected healthcare system. This project investigates current United States MIH-CP legislation in effort to provide Indiana Department of Homeland Security with updated regulations to aid in formulating future MIH-CP policy recommendations. Information was collected by primary literature review of state legislation. Data collected was based off 3 criteria: whether the state mentioned MIH-CP in their regulations, were educational standards outlined and the details of their requirements, and was it possible to bill for MIH-CP services. Numerical data that was collected was analyzed by comparing the mean and median of the applicable states. There are currently 24 states that mention MIH-CP in their state legislation but far more states have the practice occurring without regulations. Of those 24 states, 9 of them have outlined requirements for what a para-medic must accomplish in order to earn the title of Community Paramedic. Within these states there is great variance in the minimum standards of educational requirements ranging from 10 hours up to 300 hours. There was a mean requirement of 68.4 hours and a median requirement of 100 hours. Although 24 states have regulations for MIH-CP, only 8 states have laws that allow some kind of reimbursement for a community paramedic’s service. MIH-CP’s role in the EMS system is just beginning to be realized but current legislation is lagging behind. Without clear guidelines and regulations, EMS agencies must overcome obstacles that are slowing progress and hindering care. It is up to state legislators to capitalize on the potential of MIH-CP by enacting clear regulations that allow for a patient centered and interconnected healthcare system to emerge.

- Background: Zone based photodynamic therapy (PDT) is a promising treatment option for actinic keratosis, superficial basal cell carcinoma, and in-situ squamous cell carcinoma. PDT has similar cure rates to 5-fluorouracil and imiquimod. Objective: Determine if PDT zone treatments improve patient comfort, tolerance, retention, and offer less severe reactions as compared to full face treatments. Methods: 500 PDT patients were divided into full face (n = 250) versus zone treatments (n = 250). All subjects completed a post-treatment phone survey assessing tolerance of procedure. - This project, OPTIMISTIC, is a retrospective study that focuses on Congestive Heart Failure(CHF) in Indiana Nursing homes. The purpose of the study was to find better ways in which to implement in house care to these CHF patients rather than transferring them to the hospital. There were a variety of data analytics tools, primarily IU REDCAP, uses to pull charts from the OPTIMISTIC nursing homes in Indiana. Five of the nursing homes were selected and used for this study. All five nursing homes were travelled to and the on-campus charts were reviewed as well as any online charts they used in their facilities. A data collection tool was used to collect specific data for each patient that was used in the study. The results indicated that while CHF is a very hard disease to manage the Nursing homes were doing an adequate job of treating them in house. 53.3 % of the patients returned to pre-certification condition and remained in the nursing home. Most nursing homes in the study followed the OPTIMISTIC program in which they were prescribing a diuretic as the first line agent. In conclusion, the OPTIMSITC program is decreasing hospital transfers within the Indiana nursing homes as well as saving health care dollars. In the future focusing a research project on the art of charting a patient in a medical database could be very helpful. The discrepancies between the nursing home charts brought forward a major problem in the medical industry as a whole.

- The Transforming Growth Factor-beta (TGFβ) superfamily is a large group of signaling molecules whose members play conserved roles in embryo specification, organogenesis, and tissue homeostasis. Structural considerations delineate the TGFβ superfamily into ligands that interact with ALK1/2/3/6 or ALK4/5/7, with the former being mostly BMPs and the latter being mostly Activins and TGFβs. Reports of distinctly opposing effects between the BMP/GDF and Activin/TGFβ pathways are becoming increasingly common. A major focus of our laboratory is elucidating the molecular mechanisms that allow the BMP/GDF and Activin/TGFβ pathways to antagonize one another. To accomplish this goal, we must create a first-of-its-kind biosensor reporter system (called the Pathway Ratio Reporter System, or PARRES) that will allow us to examine the real-time ratio of BMP/GDF vs. Activin/TGFβ in vivo. PARRES is a novel biosensor system that uses two independent fluorescent reporters that are linked through reciprocal RNA interference so that, through mutual antagonism, fluorescent signal occurs only when transcriptional activation through one pathway is dominant. Here, we focus on the crucial, initial characterization of PARRES – the maturation and half-life kinetics of the fluorescent proteins. Using HEK293 cells, we demonstrate that the maturation kinetic of the Emerald variant of Green Fluorescent Protein is acceptable but the half-life is far too long for our needs. We subsequently destabilize Emerald to an approximate half-life of 29 hours by appending a validated degradation domain. Current studies involve generating point mutations in the destabilized Emerald protein to further reduce its half-life. Once completed, this biosensor will be especially useful to our lab as a tool for examining the influence of canonical signaling by skeletal mesenchymal progenitors, a population known to rely on BMPs, GDFs, TGFβ and Activins to instruct their differentiation into tissue-specific cell types such as osteoblasts.

- Coxiella burnetii is an obligate intracellular bacterium and causative agent of culture-negative endocarditis. Although Coxiella initially infects alveolar macrophages, it is found in lipid droplet (LD)-containing foamy macrophages in endocarditis patients. LDs are host lipid storage organelles containing cholesterol esters (CE) and triacylglycerols (TAG). Our previous studies show that Coxiella actively manipulates host LD metabolism via its Type 4 Secretion System (T4SS), which secretes bacterial effectors in the host cell cytoplasm to manipulate cellular processes. Further, specifically blocking adipose triglyceride lipase (ATGL)-mediated LD breakdown inhibits Coxiella growth suggesting importance of LD-derived lipids for bacterial growth. However, how Coxiella regulates LD breakdown and the composition of LD-derived lipids is unknown. Our preliminary fluorescence microscopy studies using CRISPR knockouts and LD inhibitors indicate presence of TAG-rich LDs in Coxiella-infected cells. ATGL-mediated breakdown of TAG-rich LDs releases arachidonic acids, precursors for lipid immune mediators important for immunomodulation during bacterial infections. Hence we hypothesize that Coxiella manipulates ATGL via its T4SS to initiate TAG-rich LD breakdown and subsequently modulate the immune response to promote bacterial survival. To test this hypothesis, we analyzed ATGL gene expression in differentially infected cells using qRT-PCR. Compared to uninfected and T4SS-infected cells, Coxiella infection increased ATGL expression indicating T4SS-dependent regulation of ATGL. Ongoing studies are elucidating the Coxiella T4SS-ATGL interaction. To identify cellular CE and TAG levels and the breakdown products at different times post-infection, we are performing thin layer chromatography (TLC). Completion of our studies will identify the LD breakdown-derived lipids and how Coxiella regulates LD breakdown of to promote its intracellular survival.
