- The COVID-19 pandemic resulted in widespread mRNA vaccinations against SARS-CoV2. Side effects include fatigue, headache, myalgia, fever, nausea, and lymphadenopathy, frequently experienced in younger recipients (16-55 years) following the second dose. Hemophagocytic Lymphohistiocytosis (HLH) is a rare syndrome of uncontrolled systemic inflammation identified by clinical and laboratory signs of immune activation. In pediatric patients, HLH occurs in 1.2 cases per million persons per year. If untreated, HLH is often fatal, with a median survival of 2 months.

- Introduction: The Catholic medical ethic is supported by a rich intellectual tradition and remains foundational to the practice of medicine, especially within Catholic hospitals. In the US, the United States Conference of Catholic Bishops’ "Ethical and Religious Directives for Catholic Health Care Services" (ERDs) is the cornerstone for practical application of ethical principles in a healthcare setting. A major barrier to said application remains, due to a lack of education regarding the ERDs and formal ethical reasoning. Objective: The purpose of this project is to create and maintain an application that presents common ethical dilemmas and walks the user through them by asking pertinent questions to elicit the central ethical questions. The goal is to inform the medical conscience and teach ethical reasoning. Methods: The application is organized in like manner to the ERDs with three distinct parts: Beginning of Life, During Life, and End of Life. Within each part are various relevant scenarios. Selecting a scenario begins a series of ‘yes or no’ questions aimed at guiding the provider through the unfamiliar waters of ethical decision making while they steer the ship with their medical knowledge. With the specific medical details relevant to the case, the provider ‘fills in the blanks’ and the application is able to make a moral recommendation based on scenario and the ethical tradition of the Church. Each question contains relevant citations/links and comments that provide further reasoning regarding the question and why it is important in getting to the crux of the matter at hand. Results: The application is currently available as "Catholic Medical Conscience App" on iOS and Android platforms. It features an easy to use interface that includes the ability to search by topic. This tool is free to use and capable of being updated and expanded upon by medical ethics experts. It has received a nihil obstat by Fr. C. Ryan McCarthy, censor liborum of the Archdiocese of Indianapolis. Conclusions: The application is a robust and adaptable tool for guiding physicians in ethical decision making for real-world scenarios. Backed by the immense moral tradition of the Catholic Church, it provides easy to follow logic and supplementary re-sources to not only parse particular questions, but also delve deeper into medical ethics in general.

- Head and Neck Squamous Cell Carcinoma (HNSCC) is one of the most common cancers in the world, and even in developed countries like the United States, the five-year survival is only 65%. This already low survival rate drops precipitously for metastatic disease, but unlike other malignancies, even regional spread significantly decreases 5-year survival. Despite its devastating impact on patient outcomes, conventional modalities to detect local micrometastases in oral cancer are inadequate and often debilitat-ing. Therefore, it is critical to identify more accurate means of predicting metastasis from the primary tumor. Oral cancer is unique in its ability to maintain intratumoral vasculature and several studies have linked increased intratumoral vascularity with in-creased risk of metastasis in oral cancer. However, immunohistochemical analysis of microvessel density alone is insufficient to predict the metastatic propensity of the tumor accurately. As an alternative, it may be possible instead to identify the molecular drivers of this vascularized phenotype and provide a more robust statistical risk assessment. Using an in vivo model of oral cancer metastasis, we have identified Chro-mobox 5 (CBX5) as a putative regulator of intratumoral vascularity and downstream metastasis. CBX5 is an epigenetic regulator that is mainly associated with chromatin remodeling and heterochromatin formation. Loss of CBX5 has been associated with more aggressive tumor types, suggesting that it may epigenetically regulate the tumor microenvironment. Indeed, CBX5 expression is significantly increased in avascular metastatic oral cancer cells compared to those cells that give rise to a vascular tumor phenotype in vivo. We have further hypothesized that overexpressing CBX5 will inhibit the in vitro metastatic phenotype of these cells. We will differentially regulate CBX5 expression in these cells through knockdown and overexpression and subsequently examine their migration, invasion, and motility in vitro. By confirming a mechanistic role for CBX5 in the regulation of a vascularized, metastatic phenotype, we will contribute to the development of a minimally invasive and reliable diagnostic technique that can improve outcomes for oral cancer patients.

Curriculums, content delivery, and procedures across medical schools and governing bodies worldwide had to rapidly change to alongside rapidly adapting guidelines to ensure continuity of education for the future physicians of the world.

- Troglitazone (TRO), a peroxisome proliferator-activated receptor (PPAR) agonist, was a drug used as an anti-inflammatory and anti-diabetic agent. We had previously investigated a PPAR agonist that induced apoptosis in dose dependent manner in our human colorectal cell line HCT116. Our current experiments are designed to determine if TRO is able to induce apoptosis, and if this action is mediated through the PPAR receptor subtypes. PPARɣ specifically is a known member of the steroid receptor superfamily and is a ligand-activated nuclear transcription factor that regulates lipid and glucose homeostasis. As a PPARɣ activator, TRO, a thiazolidinedione, has been shown to have the effect of inducing cell differentiation but also inhibiting cell growth, proliferation, and angiogenesis thereby facilitating apoptosis, making it a potential candidate to study as a cancer chemotherapeutic agent. Through the use of flow cytometry, TRO-induced apoptosis of tissue culture grown colorectal cancer cells was examined. Apoptosis was quantified by analyzing cells that contain less than the 2N DNA content (one hallmark of apoptosis). A dose response curve was determined for increasing concentrations of TRO vs sub2N DNA content. These studies confirm that TRO was able to induce apoptosis in our cell lines at concentrations greater than 30 M. Next we wanted to determine which PPAR receptors subtypes are being activated by TRO. HCT116 cells were exposed to various concentrations of TRO for 24hr then RNA was isolated from these cells. Specific gene targets to each PPAR receptor subtype were identified and using RT-PCR, changes in mRNAs expression was examined. Targets PPAR were consistently induced, PPAR were not induced, and PPAR were inconsistently induced. We are working to refine these experiments to get better determinations on which receptors are activated by TRO. These experiments confirm that TRO induces apoptosis in HCT116 cells. Additionally, we have identified changes in at least one target of PPAR receptors in response to TRO exposure. Further experiments are needed to exactly determine which PPAR receptors are being activated by TRO, and whether the apoptosis medicated by TRO is a result of PPAR receptor activation.

- Hypoxia is a critical stress that can affect the normal development of a mammal embryo. Normally, Hypoxia is buffered by the Utero-placental apparatus in human infants; however, prematurely delivered infants may encounter developmental defects due to the loss of the controlled environment of the uterus. Therefore, understanding the function of the genes that are hypoxia-induced is critical to understanding developmental injury in order to attempt to prevent or treat it. Why zebrafish as a model? Experimenting with zebrafish has many advantages: Approximately 2/3 of its genome is conserved with Humans, Zebrafish embryos can tolerate a wide range of hypoxia and the embryos develop ex-utero, which allows precise manipulation of the hypoxic environment and observation of phenotypes.

- Under the Hospital Readmission Reduction Program (HRRP), hospitals with high readmission rates are penalized by reductions in Medicare reimbursements. In particular, Parkview Noble hospital has experienced high readmission rates for chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), and acute myocardial infarction (AMI). Parkview Noble’s combined 30-day readmission rates for these conditions is 18.19% compared to the national average of 15.86%. Objective: In order to effectively reduce the readmission rate for COPD and CHF patients at Parkview Noble, this study examined the socioeconomic and clinical factors that can be used to target this patient population for healthcare driven interventions. Design/Methods: Data was collected in the form of a retrospective chart review and phone interview of 260 patients. The data obtained was then analyzed using student t-test, chi-square test, and multi-variable regression to determine statistically promising variables. A linear regression using statistically promising variables was performed to obtained preliminary predictive algorithms for COPD and CHF readmission. The sensitivity and specificity of these equations was then plotted using various cut-off values to determine their practical effectiveness. Results: The statistical analysis determined four factors with strong predictive value including: past ED visits in the last 6 months (p=1.4E-6), past ED admissions in the last year (p=0.03), heart failure with coronary artery disease (p=0.05), and stage of COPD (p=0.06). The sensitivity and specificity plots suggest that it is possible to target 20% of readmission patients while still maintaining over 85% specificity. Conclusions: From this preliminary data it is seen that several variables have value in determining a patient’s likelihood of readmission. Using this data as a benchmark, this study will be expanded to include up to 1,028 more patients before a final predictive algorithm is computed and tested.

- Gardnerella vaginalis (GV) is associated with bacterial vaginosis (BV), a dysbiosis that predisposes to preterm labor and increased susceptibility to sexually transmitted infections. The potential role of cytokines elaborated by the vaginal epithelium may impact BV symptoms and hence, is the topic of this research. We hypothesized that host-derived chemokine expression will correlate with Nugent (Gram stain species diversity) severity scores obtained from women with GV colonization. This was tested in a co-culture model of human vaginal epithelial cells (VECs) exposed to various clinically isolated strains of GV. These strains were obtained from ATCC/BEI as follows Two additional clinical isolates of GV (strains 14 and 15) with unknown Nugent scores were also obtained. To further characterize the strains, clade analysis was performed by PCR. VECs were grown in KSF media and GV strains were grown on V-agar, all incubated at 37º C in 5% CO2. For co-culture, 10 µl aliquots of each GV strain (obtained from an overnight culture) was inoculated into confluent cultures of VECs; uninfected VECs served as control. After 24 hours, culture medium was collected, centrifuged, and 500 µl aliquots of the supernatant was applied to a chemokine assay membrane (Proteome Profiler Human Cytokine Array R&D Systems chemokine profiling was performed according to the manufacturer’s protocol which tested for 36 analytes. Membrane profiling showed that 23 cytokines and chemokines were variously upregulated following exposure of VECs to GV, while others are down-regulated relative to the control. Continued investigation will attempt to correlate Nugent scores and placement of strains into clades 1-4. Our results indicate that co-culture with GV induces release of numerous chemokines from VECs. Future work will determine if these chemokines signal in an autocrine/paracrine manner and/or participate in the recruitment and activation of immune cells in response to GV colonization.
