- Passing of the TORCH: A Medical, Historical, and Social Comparison between Rubella and Zika

- A historical analysis and comparison between rubella and zika reveals that neither virus became prominent until their respective congenital syndromes were classified. Rubella was first described in the 1750s but took until 1941 when Norman McAlister Gregg characterized Congenital Rubella Syndrome to become medically significant. Similarly, zika virus was discovered in 1947 but only made headlines in 2016 when its infection during pregnancy was associated with fetal microcephaly. An investigation into the response levels that each virus received following the discovery of their respective congenital syndromes reveals a fascinating correlation. Despite zika being primarily a disease of the developing world and rubella historically being a worldwide disease, with a previously high prevalence in developed countries such as the United States (US), zika was found to have been studied and controlled at a much faster rate than rubella. This correlation is further highlighted by the fact that there are currently no available therapies or vaccines for zika. Using the US as a benchmark, the rubella vaccine was available in 1969 following the rubella epidemic of 1964-1965. However, it would take until 2004 before the US was declared rubella free. When compared to zika, following the 2015-2016 outbreak in the Americas, the incidence of zika in the US went from 10 cases in 2015 to a staggering 36,512 cases in 2016. Remarkably, by the end of the following year, the incidence had dropped to just 666 cases in the states. In 2018, there were only 148 zika cases in the US. A review of Thomas McKeown’s work on population growth, the McKeown Thesis, provides insight into the US’s successful zika control. The Thesis posits that broad-based social efforts at the population level are more significant at affecting public health than narrow-based medical interventions at the individual level. The swift control of zika despite the lack of specific therapeutics suggests the McKeown Thesis’s applications. This presentation provides an in-depth analysis and comparison between the medical, historical, and social components of rubella and zika to demonstrate their ongoing implications and influences on society. This presentation will reflect on the progress and history of medicine within the past century and demonstrate the need for continued vigilance within the medical community.

- Eccrine sweating glands are divided into functional units: I) a bulbous secretory coil that generates precursor fluid, and 2) a reabsorptive duct that transports the fluid to the skin surface. The secretory coils reside in the deep dermis or hypodermis and consist of clear cells, dark cells, and myoepithelial cells. Clear cells are the primary serous secreting cells and the likely source of secretory disorders of the gland. Dark cells are responsible for exocytosis of many of the non-ionic constituents found in sweat and are associated with certain anhidrotic sweat glands, and in addition have secretory components of their own. Myoepithelial cells surround the secretory coil and contract to provide the structural rigidity necessary to generate pressure within the gland. The reabsorptive duct contains a two-cell thick structure that modifies the fluid being transported to the skin surface.

- The ability to communicate with patients effectively continues to be a major hindrance for physicians and physician offices. Coordinating care via conventional telecommunication, involving making calls to a healthcare facility or call-center for appointments and follow-up. is often fraught with the difficulties of long wait times, which in tum discourages patients' confidence in obtaining timely and appropriate care. Despite this. the prevalence of call centers in medical practices is predicted to increase within the next decade2. The advent of numerous technologic advancements and software solutions have offered opportunities to enhance the accessibility of healthcare providers with their patients. Specifically, multiple mobile device "apps" have sprouted in the healthcare field, specifically designed to be HIPPA compliant and utilized by providers). Various EMR-linked patient portal applications nave shown to be well received and helpful in avoiding unnecessary ER and office visits-4. While many of these systems allow for updates to be posted on patient's computer or mobile devices, research into the effectiveness and practicality of smartphone exclusive applications. utilizing direct-to-provider texting features is lacking, We investigated patient preference using a smartphone, HIPPA compliant healthcare texting app, compared to conventional telecommunication with the office. in perceived quality of care. as well as surveyed the impact that these interfaces have on preventing unwarranted ER visits.

- Context: Patient reminder notifications have been widely utilized in the healthcare setting for several decades. Some clinics have even begun implementing “arrival time” notifications in lieu of the term “appointment time” in an effort to improve patient punctuality. There is little research on how these oft-used terms and practices are perceived by patients. Objective: The goal of this study was to identify patient perceptions of key event terminology, and the impact of these perceptions upon reminder system preferences. Design: We designed a paper-based survey to investigate patient perceptions of “appointment time” and preferences regarding proposed implementation of a reminder system in which an “arrival time” was provided instead of an “appointment time”. Setting: IU Health Methodist Family Medicine Center is an urban, academic primary care clinic located in Indianapolis. Survey responses were collected from 524 patients over 10 clinic days. Methods: The survey was piloted with several patients to ensure that the greater population could understand each question. Statistical methods included frequency and regression analyses. Results: The perception of the term “appointment time” varied among patients--50% interpreted it as being the time at which to enter the clinic; 14% believed it to be the time when the doctor enters the exam room. The proposal of a clinic implementing an “arrival time” reminder received significant satisfaction scores among the patient population that initially perceived “appointment time” as being earlier in the visit process (p = 0.02). Qualitative analyses of psychosocial and behavioral associations are in progress. Conclusions: There is lack of a standard definition for the term “appointment time” in the healthcare setting; perception of the term varies widely among patients. The relationship between patient perceptions of key events, punctuality, and clinic flow should be further explored before altering current reminder practices.

- Interprofessional education or IPE involves bringing individuals from two or more professions together to learn and communicate to improve future teamwork and collaboration. IPE research has shown that exposing future healthcare professionals to interprofessional training results in positive patient outcomes and reduced institutional costs. Our study will be a case based interprofessional simulation between Marian University nursing and medical students. The goals of our study include: 1) evaluating the perceptions of interprofessional education among nursing and medical students and 2) examining whether there are beneficial areas to overlap the nursing and medical training curriculums to implement effective IPE opportunities. The study will be conducted on January 22, 2020 in the Simulation Center at MU-COM, under the direction of David Patterson. Participants will be recruited via flyers and email communication. Incentivization could include using Amazon gift cards, funded via research funds available to BMS graduate students. Prior to the simulation, participants will be asked to complete an online module via Canvas. The module will pertain to the case, include anonymous demographics questions, as well as, a pre-survey. The Interdisciplinary Education Perception Scale (IEPS) survey will be used for pre and post simulation. Each participant will randomly be assigned a simulation team, balancing nursing students with medical students. On simulation day, participants will undergo a brief introduction/orientation to the required equipment. Once they are oriented to their surroundings they will work through the case as a team. The chosen case, which involves women’s reproductive health, will require students to not only use their clinical skills to treat the patient, but will also require them to handle a religious social aspect that will affect the available treatment options. When the teams have come to a conclusion with the case, they will enter a small conference room to undergo a debriefing session where facilitators, including both nursing and medical school faculty, will help them analyze their use of communication during the simulation. After this short discussion, participants will again take the IEPS survey, as well as provide feedback on the simulation.In the post-simulation analysis, we will compare pre-survey scores with post-survey scores and compare how perceptions of interprofessional education have changed. We believe that perceptions of IPE will improve from pre-simulation to post-simulation and that the participants will feel better prepared for future collaboration.

Why is this an issue? • Prices of newly marketed and pre-existing pharmaceuticals in the United States has exponentially increased each year. • The United States has spent more on pharmaceuticals than other countries. (PGPF) • Increase in co-payments for pharmaceuticals reduce affordability of prescribed regiments and reduce patient adherence. (Kesselheim et. al.) How did the issue come to be? A series of instances have led to the issue at hand such as: • United States healthcare system enables pharmaceutical manufacturers possessing the ability to freely set prices after approval. (Rome et.al.) • The United States possesses market forces that enable continuous high pharmaceutical prices: protection from competition and negotiating power. (Kesselheim et. al.). - In the United States, one of the biggest issues that every state is experiencing is the shortage in doctors. Primary Care Physicians make up a huge percentage of the shortage in doctors. The Midwest took a huge hit by this shortage, especially in rural Indiana. In Indiana today, there are approximately 3,900 primary care physicians. Most primary care physicians are located more towards bigger cities such as Indianapolis, but due to the high demand for primary care physicians, it is difficult to see a primary care doctor. According to pulse today, a primary care physician sees on average more than 25 patients a day and it is believed to increase if this shortage is not addressed in the next decade. - Children with acute lymphoblastic leukemia (ALL) suffer a litany of therapy induced side effects. Methotrexate is a common chemotherapy agent administered for the treatment of ALL. Failure to clear methotrexate from the body can result in toxicities including mouth sores, mucositis, and organ dysfunction, which have been well documented in the literature. To date, no studies have examined the correlative relationship between delayed methotrexate clearance and constipation. This study aims to examine chemotherapy induced constipation, current management practices, and the relationship between delayed clearance of methotrexate and constipation.

- Background: It has been observed that patients with poor mental health are relatively frequent users of the Emergency Departments (ED). The objective of this study was to evaluate the prevalence of numerous behavioral health domains (depression, anxiety, PTSD, substance abuse, and suicidality) in patients presenting to the IU Health Methodist Emergency Department. Methods: This study seeks to enroll 1000 English-speaking adults presenting to Methodist and Eskenazi ED. Patients were assessed for behavioral health problems using the CAT-MHTM, PHQ-8 and GAD-7 screening tools. Data on disposition medical history, discharge diagnoses, and ED utilization in the 12 months before and 30 days after enrollment from EMR and INPC will be reviewed. Results: 375 patients have been enrolled. Of those 59.4% were female with an overall mean age of 46.1 (SD ± 16.4). CAT-MH results showed 216 (57.8%) had no depression, 158 (42.2%) had mild-severe depression, while 263 (70.3%) had no anxiety and 111 (29.7%) had mild-severe anxiety. Inter-rater reliability for CAT-MH, GAD-7 and PHQ-8 was about 0.80 for a 95% CI. Conclusion: In our sample, almost half of patients that visit the ED have screened positive for mental health problems. We believe that early identification and appropriate referral may reduce inappropriate ED utilization.

- The United States spends well over $100 billion for millions of people affected by Alzheimer’s disease and related dementias (ADRD) and for their family caregivers. Most people with ADRD have multiple chronic comorbid conditions requiring help from family members for proper management. A lack of optimal care for the cognitive, behavioral and functional needs of persons with dementia further compounds the challenge of managing chronic comorbid disease. Family caregivers of persons with dementia have many unmet needs as well. This adversely affects caregivers’ ability to manage the chronic medical conditions of the person with dementia and may explain the greater use of emergency departments (EDs) and more return visits for those with dementia compared to those without dementia. How best to support family caregivers in preventing the need for emergency care has not been tested. The goal of this study is to test whether a novel care management program for family caregivers of persons with dementia can reduce the need for ED use. We will also test whether this intervention will lead to more activated caregivers who may also benefit by having fewer symptoms of depression and anxiety, and experiencing greater social support. This study focuses on family and friends who serve as caregivers of people with probable dementia who reach the crisis of needing emergency department care. The proposed intervention, the Program of Intensive Support in Emergency Departments for Care Partners of Cogni-tively Impaired Patients (POISED-CPCIP), will start in the emergency departments of two medical centers at academic institutions, New York University (NYU) and Indiana University (IU) and will recruit family caregivers and their care recipients (dyads) among patients who perform poorly on cognitive testing (indicating a high likelihood of dementia) and who are going to be discharged to home. We will randomize dyads to either the POISED intervention or to usual care. POISED will use typical dementia care management procedures provided by nurse care managers and care manager assistants who will additionally employ an intensive focus on understanding and supporting the needs of chronic disease management and on caregiver activation. In POISED, we will apply the principles of root cause analysis, a well-known quality improvement technique (that was started in the NASA space industry and later applied to health care) to understand what led to the ED visit and then apply what is learned to six months of care management. If this randomized controlled trial demonstrates reduced rates of ED use and a decrease in caregiver psychosocial symptoms, we will have validated a scalable program for supporting caregivers of people with dementia that should lead to significant healthcare cost reductions while providing much needed support for persons with dementia and for their family caregivers.
