There are limited data regarding research experience and/or interest among osteopathic medical students despite a rapidly increasing enrollment and expansion of the number of osteopathic medical schools. A 2016 study of first-year osteopathic medical students at WesternU/COMP and WesternU/COMP-NW indicated that 81% of respondents had prior research experience and 75% were either currently doing research or were interested in doing research during medical school. Here, we extended that survey to Marian University College of Osteopathic Medicine to determine the research experience and interest among first-year osteopathic medical students. Based on a response rate of 32%, a majority of respondents reported prior research experience (79.7%), with 13% of those students having garnered peer-reviewed publication(s). This is consistent with a strong perception of research being valuable, with 96.43% indicating some level of importance and 41.07% indicating "very" or "extremely" important. Interestingly, fewer respondents (56.6%) are either currently participating in research or affirmed interest in performing research during medical school, with the highest level of interest in clinical research (32%) followed by basic science (22.67%). Regarding incentives that might encourage participation in research, respondents prefer monetary compensation (37.38%) and/or extra credit in courses (36.45%). Reported barriers to performing research include possible negative impact on coursework (59.04%) and preference for other extracurricular activities (18.07%). Most students (78.57%) reported awareness that research opportunities are available, but fewer (46.43%) were aware of whether opportunities exist in their specific field of interest. Our findings indicate a strong positive perception of research among MU-COM students and highlight opportunities for improved communication and enhancement of the research environment through incentivization and/or removal of perceived barriers.

There is a lack of data regarding real and perceived barriers to research participation among osteopathic medical students (OMS’s). We have previously addressed this open question by surveying incoming first-year OMS1’s and found that the possibility of negative impact upon performance in coursework was the predominant concern among respondents at MU-COM. Here, we held a series of three focus groups with third-year OMS’s (n=23), which have completed preclinical training, to gather current perceptions and evaluate how those perceptions may have shifted throughout their preclinical experience. Focus group sessions were facilitated by two OMS2’s and audio recorded for transcription; participants were anonymized and the transcripts analyzed for themes. Participation was voluntary and incentivized by a raffle for Amazon gift cards. This study was deemed exempt by the MU Institutional Review Board. Focus group participants expressed that the importance of research relied heavily upon intrinsic motivations for research or the desire to further their career via establishment of a professional network with their mentor or improving competitiveness for residency positions. Unlike our prior survey findings among incoming OMS1’s, OMS3’s reported monetary compensation was of little importance and possibly would discourage them from performing research due to the perceived added pressure that money would generate. Some perceptions, however, remained consistent throughout the medical training thus far. Both incoming and post-preclinical students placed a great deal of value upon time and establishing themselves academically. This proved to possibly preclude students from undertaking research. Another robust theme observed between both incoming OMS1’s and OMS3’s is the strong desire for clinical research opportunities. This longitudinal approach to research perceptions will prove valuable in enacting change to the research culture and opportunities provided here at MUCOM.

Phosphorus equilibrium in the body is regulated through uptake and release via the intestine, kidney and bone. These processes are coordinated through the activity and regulation of Fibroblast Growth Factor 23 (FGF23). FGF23 is primarily produced in osseous structures by osteocytes and osteoblasts. Under normal physiological conditions, elevations in serum phosphorus or active 1,25dihydroxyvitamin D stimulate tran-scriptional upregulation of FGF23 and promote the release of the full-length intact FGF23 protein which targets it to downstream tissues. Studies in rare heritable forms of phosphate dysregulation revealed novel pathways of FG23 regulation including iron deficiency and inflammation. Notably, FGF23 expression under these conditions were not confined to the skeletal tissues. Thus, this review will examine the regulation of FGF23 in normal and altered metabolic states as well as how they affect expression within extra-skeletal tissue.

Stimulus- independent pain behavior was also evident in both vehicle and BMP-2 treated CSD animals as defined by asymmetric directed behaviors including flinching, licking, lifting, and shaking of the ipsilateral hindlimb. Conclusion: The data indicates that post-fracture pain thresholds are similar between both healed and unhealed, young and old mice. Therefore, the effect of musculoskele-tal injury and/or buprenorphine may be responsible for the pain experienced following trauma. Distinguishing between these will be critical for further understanding how to best treat acute and chronic pain associated with musculoskeletal injury, particularly for geriatric patients.

BACKGROUND: With an aging world population, there is an increased risk of fracture and impaired fracture healing. Enhancement of angiogenesis is one potential mechanism for improved fracture healing. The question then becomes: How do therapeutic targets improve these angiogenic and vasculitic conditions to facilitate successful fracture union? The current therapy, bone morphogenetic protein 2 (BMP-2), while successful in promoting fracture union, presents with several potential side effects, including excess bone growth and an increased risk of developing cancer. We hypothesize that thrombopoietin (TPO) may serve as a novel therapeutic modality for improved angiogenesis and fracture union. Here we characterize and stimulate endothelial cells (ECs) with TPO in vitro. OBJECTIVE: We hypothesize that thrombopoietin (TPO) may serve as a novel therapeutic modality for improved angiogenesis and fracture union. METHODS: Five young (3 month-old) and 5 old (22-24 month-old) C57BL/6 mice were euthanized and bone marrow was collected from the tibia and humeri for EC isolation. Additionally, five young mice underwent a 2 mm femoral diaphyseal defect surgery. Three weeks post-surgery mice were euthanized and bone marrow from the ipsilateral and contralateral tibia as well as the humeri were collected and ECs isolated. All ECs were assessed for vessel-like formation and proliferation. All in vitro studies were completed at least 3 times with triplicate samples. Statistical analyses were performed using a two-way ANOVA with Tukey’s post-hoc testing. All procedures were approved by the Indiana University IACUC. RESULTS: In vitro characterization of ECs showed impaired vessel-like formation, branch length, number of nodes, mesh area, and proliferation in old ECs compared to young ECs. ECs isolated from the ipsilateral tibia of mice undergoing a SBD surgery had impaired ECs vessel-like parameters and proliferation compared to ECs from the contralateral tibia. Finally, treatment of old ECs with TPO improved vessel-like parameters and proliferation compared to untreated cells. CONCLUSIONS: Although preliminary, our results confirm that EC proliferation and vessel formation are impaired in aging and in the ipsilateral tibia of mice, which underwent a femoral SBD surgery. Although BMP-2 is used by orthopaedic surgeons for bone healing, its use is not without side effects, leaving researchers to investigate alternative treatments. We have previously shown that TPO is effective for bone healing. Here we show that TPO improves vessel formation in aged ECs, and therefore, TPO may improve angiogenesis and fracture healing in old mice, which is currently under investigation.

In the late 1990s, doctors were told by pharmaceutical companies that opioid pain relievers were not addictive and that they could prescribe them freely to patients in severe pain. As a result, doctors began prescribing opioid pain medications at a much higher rate. People began abusing these medications and the misuse of prescription and non-prescription opioids grew at an alarming rate. In 2017, the United States Department of Health and Human Services (HHS) declared the opioid crisis a public health emergency. Since then, opioid use and opioid related deaths have continued to increase prompting concern from government officials, healthcare providers, and public health advocates (HHS, 2023). Here we explore the current impact of the opioid epidemic and the current intervention strategies as well as explore using exercise as a complementary treatment for recovery. Submitted as part of the BMS-515 Public Health and US Healthcare System course for MUCOM Research Day.

Context: Prior studies have identified significant health benefits and socioeconomic barriers to fruit and vegetable consumption. Objective: This study describes characteristics and physiological outcomes relative to the fruit and vegetable consumption of adolescents. Design: A new analysis of the Project Healthy Schools Registry containing data from questionnaire responses, medical screenings, and blood tests was performed. Setting: Data from students in 67 middle-schools in Michigan from who were enrolled in Project Healthy Schools and completed an optional health behavior questionnaire and physiological screening were analyzed (n=2813). Methods: Students were divided into two groups based on the number of servings of fruits and/or vegetables they consumed the previous day: “high consumers,” ≥ 3 servings or “low consumers,” < 3 servings. Demographics and physiological measures were compared between groups using one-way ANOVA tests. Results: 1457 (51.8%) students were high consumers. High consumers were less likely to be Black (23.8% v. 31.6%, p<0.001) and more likely to be Asian (9.1% v. 4.7%, p<0.001) than low consumers. High consumers were also more likely to be from high SES school districts (45.2% v. 33.0%, p<0.001) and less likely to be from middle (25.0% v. 30.2%, p=0.002) or low (29.8% v. 36.7%, p<0.001) SES school districts than low consumers. High consumers had higher triglyceride levels (91.0 v. 82.5 mg/dL, p<0.001) and lower recovery heart rate (105 v. 108 bpm, p=0.019) than low consumers. Conclusions: Demographic and socioeconomic factors were associated with variations in fruit and vegetable consumption in this adolescent population. High fruit and vegetable consumers had lower recovery heart rates, which may indicate better health and fitness.

Introduction: It is well documented in the literature that Bitter Melon Extract (BME) inhibits the growth and proliferation of MCF-7 breast cancer cells. While the exact mechanism of this inhibition is unknown, it is possible that Protein Kinase C (PKC) and Rho Kinase (ROK) play a role in this mechanistic pathway due to their known critical roles in cell migration, division, and survival. Previous research has shown that ROK inhibition results in decreased cell viability and decreased MYPT1 (Myosin phosphatase targeting subunit-1) expression, while PKC inhibition alone does not have a significant effect. Aims: The aim of this research is to investigate the cytotoxic mechanism of action enacted by BME on MCF-7 cells and how PKC and ROK might be involved in this mechanism by building on past research to create more robust data and a better understanding of the metabolic state of MCF-7 cells under the set conditions. Methods: Glucose assays were run via a GM-100 glucometer to test for cell metabolism as a measure of cell viability on BME dose (0%, 0.5%, 1%, 2%, 5%, and 10% BME (v/v)) study samples, PKC inhibitor (GF109203x; 0.5 uM) samples, and ROK inhibitor (H-1152; 1 uM) samples. The samples used in each of the three conditions were secured from past research. Results & Discussion: BME dose-dependently increased glucose remaining in the culture media. The sample sizes for different doses of BME were as follows; at 0% (n=4), 0.5% (n=5), and 1% (n=5), while 2% (n=2), 5% (n=3), and 10% (n=3). The data suggests two possibilities; (i) with increasing BME dose, there is an increasing effect on the MCF-7 cell’s metabolism of glucose, and that this decrease in ability to metabolize glucose when in the presence of increasing [BME] could play a role in the cytotoxic mechanism of BME on MCF-7 cells. (ii) The BME kills cells and hence glucose is not used as much thus leaving plenty of glucose in the medium. Our results further showed that neither PKC inhibitor samples (n= 7) nor ROK inhibitor samples (n=11) showed significant differences in [glucose]. This suggests that the measured effect in glucose metabolism in the BME dose-dependent study samples is independent of PKC and ROK mechanistic pathways.

Gestational diabetes mellitus (GDM) is an increasingly prevalent complication of preg-nancy leading to adverse maternal outcomes for years postpartum. This primary litera-ture review was conducted to explore what complications of GDM have significant cor-relation with an increased risk for eventual onset of cardiovascular disease (CVD). Overlapping risk factors for GDM and CVD such as obesity, sedentary lifestyle, and poor diet are largely the cause for this correlation. Obesity is a commonly recognized cause of the progression from GDM to CVD. However, there remains the need to iden-tify other factor(s) that elevate CVD risk in women who have had GDM but who are not obese and who lead an active life. The literature review explores the issues that may be secondary to obesity, along with outcomes of GDM that lie outside of obesity entire-ly and remain known exclusively as a by-product of GDM. This allowed for some pos-sible explanations for the increased cardiovascular risk seen in women who had GDM but would typically not be considered high risk for CVD or GDM. GDM results in vari-ous adverse outcomes including hypertension, dyslipidemia, inflammation, endothelial dysfunction, and type 2 diabetes, all of which promote progression to CVD. These may serve as intermediate factors in the onset of CVD up to 25 years postpartum. Seden-tary lifestyle and advanced maternal age are critical factors favoring the progression of GDM to CVD. Further identification of risk factors leading to CVD would allow for pre-vention of onset of CVD following a pregnancy affected by GDM.
