81. Diagnosis and Management of Postpartum Eclampsia in the ED

- Postpartum preeclampsia refers to hypertension and proteinuria after delivery, and the occurrence of a seizure within six weeks after delivery is called postpartum eclampsia. Etiology is unknown but theorized to involve abnormal placental formation. Major risk factors include preeclampsia, multiple gestations, obesity, and 1st pregnancy. First pregnancy was the only known risk factor in this case. A 24-year-old Hispanic female (Gravida 1, two weeks postpartum) presented to the ED after a tonic clonic seizure, witnessed by her husband. Except for acquiring ß-hemolytic strep, she had a normal pregnancy and full-term vaginal delivery. Past medical and family history of seizures were negative. Her preeclampsia during pregnancy was closely monitored and did not require medication. She reported feeling short of breath prior to the seizure, which led to consideration of other causes of the seizure. The differential included postpartum eclampsia, new onset seizure, head trauma, intracranial hemorrhage, PE, and syncope. Patient was placed in high acuity on a cardiac monitor to track her BP and kept on seizure precautions. A magnesium sulfate drip was started since eclampsia was the most likely differential. Upon attempting a 6 gm bolus, she developed tachycardia so the drip was reduced to 2 gm/hr. Head CT, chest CT, and EKG ruled out the head bleed, PE, and syncope, respectively. Patient had a BP of 140/99, elevated LFTs, and proteinuria. Based on the patient’s eclamptic episode within the six week window, she was diagnosed with postpartum eclampsia. While the overall prevalence of postpartum preeclampsia has remained steady, incidence of postpartum eclampsia has fallen due to improved prenatal care and identification of preeclampsia. Despite these improvements, this case highlights the importance of providing women additional counseling on postpartum preeclampsia and hypertension management as well as the challenges in early identification of postpartum eclampsia in the ED.

- Students frequently report difficulty learning information presented at the end of long study sequences. Prior research has shown that information presented earlier in a study sequence interferes with learning of information presented later in a study sequence, but that interpolating study with testing can serve to protect against such buildup of proactive interference (PI). Nonetheless, it remains debated whether interpolated testing improves encoding or retrieval of information presented in later portions of study. In the present study (N =96), students learned five interrelated lists of words. Students were always tested on the fifth list and then again during a final cumulative test. We used a 2 x 2 design to vary whether or not students were tested on the first four lists (i.e., testing versus control) and also the rate of presentation of individual words (4s versus 10s per word). We found that learning of the fifth list was impaired due to PI in the absence of previous testing (i.e., for lists 1 through 4) at both short and long presentation rates. However, during a final cumulative test that required participants to recall all study words, non-tested participants recalled words from the fifth list as well as tested participants, but only if they had studied words for 10s. Taken together, our results indicate the PI during learning arises largely due to constraints imposed by initial memory tests (i.e., during retrieval), and that an encoding manipulation can serve to overcome PI during later tests that do not impose similar constraints on retrieval.

- Type 1 Diabetes is an autoimmune condition characterized by a deficiency in insulin due to the destruction of pancreatic β-cells. This leads to elevated blood glucose levels. The primary treatment involves daily administration of exogenous insulin, but it falls short in replicating natural insulin secretion of pancreatic β-cells, hindering effective glucose homeostasis. Optimal management of Type 1 Diabetes is achieved through insulin pumps and continuous glucose monitors. Unfortunately, the annual out-of-pocket expense for insulin can exceed $10,000, rendering it financially burdensome for individuals in a lower socioeconomic status who are dealing with Type 1 Diabetes. The objective of this poster presentation is to address the issues that people in low SES areas have when trying to obtain insulin for their T1DM and creating solutions for those issues. Submitted as part of the BMS-515 Public Health and US Healthcare System course for MUCOM Research Day.

- The design, administration, input, and evolution of an epigenetic database containing metadata across studies was created using cloud based tools with permission and blinding capacities. The database was designed to store the odds ratio (OR) of various interventions such as exercise,sleep, environmental exposure, and food consumption on health outcomes many of which work mechanistically via epigenetic modifications. To this end, existing published research on the epigenetic influences on disease and overall health promotion were evaluated and compared through a variety of normalized metrics. Research is often hampered by aggregating qualified researchers in one place to ensure continuity and interaction. Cloud based informatic tools allow for the electronic formation of such teams, while asynchronous data input creates a shared library of inputs and outputs essential for standardized database creation. Zotero was used for literature sharing and Google Docs for selective data sharing and visualization. The manual curation of primary literature to build databases with metadata is a fruitful avenue of research for those pursuing terminal degrees that sequester too much of their time for more classical contiguous lab bench work. This database is currently being created by third year medical school students at two medical schools, as well as a PhD student at a third school. A future application of this project is the creation of a smartphone application that would allow users to query certain behaviors or outcomes (e.g. running or cancer) and obtain a rank ordered report on how various interventions or therapeutics would combat a disease based on their odds ratios. The emerging role of epigenetics in modifying one's own phenotype can be leveraged with a database such as this to allow for patient executed lifestyle interventions which could increase health while lowering healthcare costs.

- Bitter melon extract (BME) is known to inhibit breast cancer cells (MCF-7) proliferation. Due to the high metabolic rate of cancer cells, we hypothesized that low glucose levels are detrimental to cell survivability and that BME exerts a cytotoxic effect by altering the capability of a cell to utilize glucose. Through this study, we seek to investigate if insulin is capable of mitigating the cytotoxic effects of BME and provide a “rescue effect” that increases cell viability, or even completely reverses the cytotoxic effects of BME.

- Bristles are mechanosensory organs in the Drosophila peripheral nervous system composed of four cells that adopt different fates and differentiate. These four cells (neuron, shaft, sheath, and socket) arise from a sensory organ precursor (SOP). D-Pax2 is a transcription factor necessary for the development of functional bristle cells, particularly in the differentiation of the sheath and shaft cells. This transcription factor is expressed in all four bristle cells during mitotic phases, but is then restricted to the sheath and the shaft cells during differentiation. D-Pax2 is controlled by a 3.1 KB upstream enhancer region, which can be further divided into two segments that drive early and late expression of D-Pax2 separately. The purpose of the research was to try to find the minimal regions of these enhancer segments that control D-Pax2 expression. Understanding control of D-Pax2 expression may help elucidate the mechanism of control of the mammalian homolog Pax2, which has a role in the urogenital tract, eyes, and possibly in kidney cell differentiation. This research was accomplished by cloning truncated fragments of the enhancer regions into the green fluorescent protein (GFP) reporter plasmid pH-Stinger to create transgenic flies that could be dissected and examined for GFP expression, which could be used as a readout for D-Pax2 expression. Drosophila nota were then stained with an anti-D-Pax2 primary antibody, which was detected with a secondary red fluorescence antibody. A fluorescence microscope was used to examine the occurrence of GFP in relation to D-Pax2 protein, and images were taken. Expression of GFP in fly lines bearing proximal, early enhancer fragments and distal, late enhancer fragments was greatly reduced as compared to the controls. The distal, early enhancer fragment showed a similar expression pattern to the control. It was found that this 700 BP segment was sufficient in driving early expression of D-Pax2.

Decreased SkBF was observed with CVAR (58±10, 59±4, and 63±8%), CVAR with DPP (30±13, 22±14 and 37±3%), and ischemia (98±1, 100±2 and 91±3% for the pilocarpine ED<sub>50</sub>, pilocarpine E<sub>MAX</sub>, and vehicle control, respectively). Despite these significant decreases in SkBF, changes in eccrine sweat rate were minimal (CVAR = 5±5, 9±4, and 8±6%; CVAR with DPP = 6±8, 1±5, and 7±6%; and ischemia = 5±7, 5±4, and 6±5% for the pilocarpine ED<sub>50</sub>, pilocarpine E<sub>MAX</sub>, and vehicle control, respectively). These preliminary data suggest that acute decreases in SkBF do not proportionally decrease cholinergic-induced sweating in human forearm skin. One interpretation of this is independent control and regulation of cutaneous blood vessels and eccrine sweat glands in humans.

Arm and leg SkBF (laser-Doppler flowmetry) was decreased by engaging the venoarteriolar response (limb lowered ~30 cm from heart level; CVAR), venoarteriolar response plus a decrease in perfusion pressure (venous occlusion by proximal cuff inflation; CVAR with DPP), and ischemia (arterial occlusion by proximal cuff inflation). Decreased SkBF was observed with CVAR (30±7 and 60±15%), CVAR with DPP (44±12 and 86±11%), and ischemia (96±1 and 99±1% for the arm and leg, respectively) during normothermia. Despite these significant decreases in SkBF, there were no significant effects on transepidermal water loss (TEWL). Heat stress increased intestinal temperature from 37.1±0.1 to 38.0±0.1°C, heart rate (ECG) from 65±5 to 93±4 bpm, arm cutaneous vascular conductance (SkBF/Mean arterial pressure*100) 52±12 to 473±85 units, and forearm sweat rate (capacitance hygrometry) from 0.0 to 0.6±0.1 mg/ml/min. During heat stress, decreased SkBF was also observed during CVAR (20±7 and 17±6%), CVAR with DPP (39±9 and 52±13%), and ischemia (99±1 and 100±1% for the arm and leg, respectively). Despite these significant decreases in SkBF, eccrine sweat rate was not significantly affected. These data indicate that a wide range of acute changes in SkBF do not appear to affect TEWL or eccrine sweating in peripheral skin, indicating that there is independent neural control and regulation of these two sympathetically controlled end-organs.

- Context: Children with SMA suffer from scoliosis, global weakness, and Thoracic Insufficiency (TIS). Normalized pulmonary function tests are inadequate when evaluating these patients for improvement in PFTs. Objective: Forced Vital Capacity (FVC) varies proportionately with thoracic height. Scoliosis severity affects this relationship: children w/Cobb relationship, while children w/Cobb >50 do not, and suffer from TIS. Spinal instrumentation (SI) partially restores this proportion. We evaluated whether FVC varies proportionately w/ stature, as represented by PIW, and if this relationship was affected by curve severity/treatment. Design: Retro Comparative Setting: Patients diagnosed with SMA at Boston Children’s Hospital with PFTs and x-rays. Methods: Bedside FVC and sitting spine X-ray in/out of TLSO and pre/post SI were evaluated serially. Moderate scoliosis was treated w/TLSO, whereas severe curves were treated by SI. Cobb, PIW and FVC @ treatment initiation were compared @ last f/u: 53 SMA pts. types: 1 (2%), 2 (53%), 3 (45%) were analyzed over 5 yrs. (SD 2.6; range 1.1-11.6 yrs). Results: For children with Cobb 50 there was no correlation (p=0.27); regardless of treatment. Linear mixed modeling for unbalanced groups and non-normal distributions comparing FVC/PIW for TLSO vs SI @ initiation vs last f/u, segregated by initial curve severity, indicate a difference in FVC/PIW for subjects >50° vs. < 50° (p=0.004) and for SI vs. TLSO (p=0.009). Conclusion: For SMA children with Cobb 50 there was no relationship, indicating PF disproportionate to stature = TIS. SI did not restore the proportional relationship between FVC and PIW.
