- Candida albicans and Gardnerella vaginalis are two common infectious organisms of the female urogenital tract. Single agent boric acid (BA) topical preparations have been a treatment for resistant gynecologic Candida and Gardnerella infections, but it is worth exploring combinations of inhibitors to determine if a more effective treatment may be developed. Our study aims to determine the efficacy of BA alone in comparison to the combination of BA and EDTA (BA + EDTA) by measuring organism growth, size, and morphology. Experiments used 12 clinical strains of Candida albicans and 7 strains of Gardnerella vaginalis. The organisms were grown in varying concentrations of BA and BA + EDTA media. Organisms were characterized by microscopy and flow cytometry. We report that the median effective dose of BA alone was greater than that of BA + EDTA in Candida strains. Microscopic evaluation revealed that exposure to BA and BA + EDTA decreased typical hyphal transformation of Candida, and cells appeared smaller in size. Flow cytometry forward scatter data of Candida grown in inhibitors revealed smaller cells than those grown in control conditions. Cell counts of organisms grown in inhibitors were also significantly decreased compared to control conditions. Incidental evaluation of autofluorescence revealed increased fluorescence of organisms exposed to inhibitors. Preliminary results of Candida strains grown in BA and BA + EDTA also revealed inhibition of virulence-associated protease activity. These data provide evidence supporting the potential use of a combination therapy in Candida and Gardnerella infections and prompt additional investigation into combination therapies with EDTA.

- Persisters are a special group of quiescent and phenotypically unique cells in a given microbial population that survive antimicrobial insults or stresses but retain properties of the original population. Persisters have been characterized and studied in bacteria; however, little work exists on evaluating the persister phenotype in yeast. Previous research with 12 clinical isolates of Candida albicans elucidated antifungal properties of boric acid (BA) as a potential treatment alternative for vaginal candidiasis. Objective: This project aimed to identify and quantify C. albicans persister colonies that may appear when exposed to BA, antifungals and BA-single antifungal combinations. Methods: C. albicans was grown in RPMI 1640 media, experimentally modified to 1% BA, 5μg/mL miconazole, 2μg/mL amphotericin B or 1μg/mL caspofungin. The combination conditions were at the same concentrations. Growth was quantified with flow-cytometry and CFUs counted on agar. Persisters were measured as a ratio of CFUs to flow events per μL. Results: Miconazole and amphotericin B both decreased percent persisters at 24h (0.92% and 1.88%) and 48h (3.74% and 2.37%) when compared to BA alone at 24h and 48h (33.34% and 119.11%) while caspofungin increased persisters at 24h and decreased at 48h (138.00% and 65.82%). Combination conditions decreased percent persisters when compared to BA and respective single antifungals alone at both 24h (BAMiconazole, 1.20%, BA-Amphotericin B, 0.46%, BA-Caspofungin, 17.11%) and 48h (1.60%, 0.43%, 13.30%). Conclusion: BA appears to have a synergistic effect on growth inhibition and persister colony formation. Further work is needed to elucidate if these colonies have a unique persister phenotype or are only survivors from antifungal insult. This work shows that BA may be a potential therapeutic to inhibit persister formation, a target separate from antifungal resistance.

- Objectives: Arch branching has never been shown to influence recoarctation after extended end-to-end anastomosis via thoracotomy, yet in each study bovine arch identification is grossly underreported. This study aims to: 1) assess chart review reliability in bovine arch identification, 2) determine recoarctation risk with a bovine arch, and 3) explore an anatomic explanation for recurrent arch obstruction based on arch anatomy. Patients and Methods: 49 consecutive patients underwent thoracotomy with extended end-to-end aortic coarctation repair at a single institution (2007-2012). Echocardiograms from these patients were reviewed for arch anatomy and compared to the echocardiographic reports. Recurrent arch obstruction was defined as an echocardiographic gradient across the repair ≥20 mmHg. For cases with angiographic images, a scaled clamping distance between the left subclavian artery and the maximal proximal clamp location on orthogonal projections was then calculated across arch anatomies. Results: Chart review identified 6.1% of patients with a bovine arch, compared to 28.6% on targeted image review. 28.6% of bovine arch patients had a follow-up gradient ≥20mmHg. Only 5.7% of normal arch branching patients had a follow-up gradient ≥20mmHg. The mean clamping index was significantly diminished in patients with bovine arch anatomy. Conclusions: Arch anatomy often goes undocumented on preoperative imaging, yet children undergoing extended end-to-end repair with bovine arch anatomy are at a significantly increased risk of recoarctation. This may be due to a reduced clampable distance to facilitate repair. These results should trigger a profound paradigm shift in preoperative assessment, parental counseling and surgical approach for children with discrete aortic coarctation. - Brexanolone (ZULRESSO ™) is a novel approach to treat postpartum depression in adult women as a neuroactive steroid similar in chemical composition to the hormone allopregnanolone. It was approved for use in adult women suffering from postpartum depression by the FDA in March 2019, with emphasis on its potential use in patients who are unresponsive to mainstay interventions. Brexanolone is the only drug currently on the market approved to treat PPD, use of Brexanolone requires patients to be enrolled in the REMS (Risk Evaluation and Mitigation Strategies) ZULRESSO™ program necessitating a 60-hour period of intravenous administration and close monitoring. The pharmacogenomics of this drug have yet to be studied but could potentially aide in determining if Brexanolone administration is an appropriate therapy for a patient. Additional clinical studies with larger sample sizes are necessary in order to evaluate the use of Brexanolone in women with PPD.

- Osteoporosis, a disease of low bone mass, places individuals at enhanced risk for fracture, disability, and death. In the USA, hospitalizations for osteoporotic fractures exceed those for heart attack, stroke, and breast cancer and, by 2025, the number of fractures due to osteoporosis is expected to rise to nearly three million in the USA alone. Pharmacological treatments for osteoporosis are aimed at stabilizing or increasing bone mass. However, there are significant drawbacks to current pharmacological options, particularly for long-term management of this chronic condition. Moreover, the drug development pipeline is relatively bereft of new strategies. Consequently, there is an urgent and unmet need for developing new strategies and targets for treating osteoporosis. Casual observation led us to hypothesize that much of the bone remodeling research literature focused on relatively few molecular pathways. This led us to perform bibliometric analyses to determine the relative popularity of bone remodeling pathways in publications and US National Institutes of Health funding of the last 10 years. In this review article, we discuss these findings and highlight several less-examined signaling pathways that may hold promise for future therapies.

- This project created the first website capable of searching published biomedical knowledge and returning a rank ordered list of actions most effective at improving health. These actions have font scaled based on the odds ratios (O.R.), which is a statistical method to measure the relation between an incident occurring (as in a disease) and the variable of interest (the input/action). This website was designed with the intent of making data more easily consumable for the average user concerned with health outcomes. The database PubMed has over 1 million new articles added annually, and a single human can not read and/or compare the importance of these disparate outcomes. As part of the website, a user can enter information from published peer reviewed studies which will store the information in a SQL database. The data is then searchable via the search box. Data can be input into or searched from the database in lay terms; however, the information is codified using synonyms of Medical Subject Heading (MeSH) terms. Search capability allows for lifestyle input, interventions, or outcomes desired with the results visually scaled based on PubMed odds ratio effectiveness. This is the first time the magnitude of scientific importance, as published in PubMed, is translated in a fashion to draw people’s cognitive attention scaled to that O.R. importance. Future work may include providing feedback (e.g. educational credit) to incentivize submissions, or natural language processing (NLP) to allow real time automated incorporation of newly deposited PubMed articles.

- Odds ratios are one of the methods in statistics to quantify how strongly the presence or absence of a property correlates with the presence or absence of another property in a given population. In the biomedical sciences they are an effective way to investigate the effectiveness of medical interventions towards desired outcomes and the strength with which certain factors influence the risk of a particular disease. Our goal is to collate the significant amount of data that has been published regarding interventions and risk factors using odds ratios into a single database that can be used by the public to compare their effectiveness and importance in influencing desired outcomes in medicine.

- Acute cardiac injury is a common cardiovascular complication of Covid19 and little is currently known about the long term implications of the complications and manifestations. Vascular complications show a hypercoagulable state which Indicates potential for thromboembollsm. By monitoring for both cardiac and vascular complications after hospitalization and administering anti-platelet and anti-coagulation therapies we have seen prevention of complications such as VTE. Developing a CV screening protocol for Covid-19 patients and recovered patients Is crucial in monitoring and further research.
