Laryngospasm and bronchospasm are anesthesia complications that occur commonly in the operating room. Laryngospam and bronchospasm can both lead to hypoxia, hypercarbia, and hemodynamic instability. Our goal was to determine if first year SRNAs would have a better understanding of identifying and treating each complication through a simulation and debrief scenario. We used a pretest to gather baseline knowledge and confidence level followed by a simulation where the subjects were to identify and treat either bronchospasm or laryngospasm. Following the simulation, a debrief was conducted to educate and discuss the simulation using an interactive powerpoint presentation. After the debrief, the same simulation was conducted again. Afterwards, a posttest was administered to determine if an increase in knowledge and confidence was gained. As a result, there was an increase in knowledge and confidence following the simulation and debrief.

Malignant hyperthermia is a rare but potentially catastrophic syndrome that can occur during general anesthesia. High mortality rates occur when this complication is left untreated or when care is delayed. Due to the rare nature of this process occurring there are few surgical staff members that have previous experience to fall back on should a MH crisis present itself. For this reason, developing a simulation for MH is a great way to prepare staff members and departments for a malignant hyperthermia event. Many facilities do not follow the recommended annual training on MH response. The purpose of this project was to establish MH simulation within the surgery department of a critical access hospital as a means of preparing the staff for a future MH event. The goal was to establish the simulation and demonstrate both an increase in knowledge gained by the staff as well as an improved confidence in ability to recognize symptoms and implement current treatment methodologies for MH. Assessment of these goals included evaluation of the staff by pre/posttests to assess learning and survey reflecting staff opinions on their confidence in handling a future live event. The results demonstrated improvements in both knowledge gained as well as learner confidence (p<0.01). Simulation is an effective method for educating operating room (OR) staff on the management of MH. Participants within the study had improved knowledge, confidence, and voiced satisfaction with the program. This serves as a potential method to implement at additional sites to encourage departmental preparation.
Methocarbamol: Effect on Postoperative Pain Following Laparoscopically Assisted Vaginal Hysterectomy (LAVH)

Background and Review of Literature: Laparoscopically assisted vaginal hysterectomy (LAVH) is a minimally invasive surgical procedure commonly performed to remove the uterus. While the LAVH technique offers many advantages, management of perioperative pain continues to be a concern. Methocarbamol, a centrally acting antispasmodic, has gained popularity by many anesthesia providers as a multi-modal pain adjunct.
Purpose: This project was developed to evaluate the effect of intraoperative methocarbamol administration on post operative pain scores in patients undergoing an LAVH procedure.
Methods: A retrospective chart review of patients who underwent an LAVH procedure was performed to analyze and compare postoperative pain scores between patients who received methocarbamol intraoperatively to those who did not receive methocarbamol.
Implementation: The medication administration record (MAR) of 80 patients was reviewed to determine if the patient received methocarbamol intraoperatively. Intraoperative and postoperative opioid administration were recorded separately. A two-sample t-test was utilized to compute significance of total opioid consumption and average post anesthesia care unit (PACU) pain scores between the two groups.
Results: The total intraoperative and postoperative opioid consumption between the methocarbamol (+) and methocarbamol (-) groups revealed no significance. Patients who received 1000 mg of methocarbamol intraoperatively had significantly lower pain scores at the 5, 15, and 30-minute time intervals (p=0.03, p=0.01, p=0.03) in the PACU.

Microlearning Effect on Perioperative Staff Performance Warming Surgical Patients: Using Surgical Patient Temperatures Before, During, and After Surgery

This quality improvement project investigated the impact of microlearning on perioperative staff behavior regarding maintaining optimal patient temperatures throughout the surgical process. This project also served as a targeted data collection initiative aimed at enhancing patient experience during surgery. High level performance in healthcare has become challenging often due to fatigue arising from volumes of information workers must absorb in short periods of time. Recent literature suggests that much of the knowledge derived from information consumed in bulk is minimally retained and quickly forgotten.
Microlearning breaks down complex information into targeted segments and facilitates focused understanding. Microlearning involves a variety of easily accessible formats, such as mobile applications or online slide presentations that are accessible to the learner for acquiring current information, effectively refresh knowledge that has become inactive, or promote learners to function at higher levels with new perspective. One focus of the project was on assessing the level of importance staff placed on monitoring patient temperature as well as their knowledge of, and adherence to, institutional policies. The educational intervention consisted of an online presentation focused on thermoregulation during surgery and facility policy.
Surveys were administered to measure staff knowledge before and after the educational intervention then analyzed via paired t-tests to measure the microlearning intervention's effectiveness on staff knowledge. The educational intervention and surveys were delivered through Qualtrics to ensure participant convenience. The second phase of this project included retrospective chart reviews to discern the impact of the microlearning intervention. To provide insight into the effectiveness of the microlearning intervention, extrapolations were performed utilizing statistical inference and physical patient temperatures before, during, and after surgery.

Needs Assessment of Podcast Use for Professional Development and Lifelong Learning in CRNAs

In the rapidly evolving world of medical education and content, professional development is not just a choice, it is an obligation. Despite the rising popularity of podcast use over the last 10 years, there is still little-known if they effectively meet the needs of professional development and lifelong learning for anesthesia experts, specifically CRNAs. A needs assessment was performed to determine if anesthesia-focused podcasts are a valuable component to professional development and lifelong learning. The needs assessment survey encompassed 14 questions that looked at participant demographics, overall outlooks and preferences on current anesthesia-focused podcasts, desired modes of professional development, and if they considered podcasts to be a valuable component of professional development and lifelong learning in nurse anesthesia. After data analysis was completed, results from the survey responses indicated that anesthesia-focused podcasts are perceived as a valuable component to professional development and lifelong learning. In-person lectures/professional meetings were the most preferred mode of professional development (75% of responses). Podcasts were the 2nd most preferred mode of professional development (52.27% of responses). Relevant (92.86%), Credible (71.43%), Quality-focused (50%), and accessible (50%) were found to be the most valuable principles of professional development and lifelong learning. Although there is no research on podcast-use as an alternative method to achieving professional development and lifelong learning, this project demonstrates how anesthesia-focused podcasts have the potential to become a valuable component to the future of the nurse anesthesia profession.

Nurse Anesthesia Program Structures Impact on SRNA Stress Levels

Understanding the effect of stress on Student Registered Nurse Anesthetists (SRNAs) during their CRNA school could improve the development or expansion of program structure and wellness interventions to help manage stress during school. Support by CRNA programs and faculty could improve coping strategies and address areas of concern that extreme levels of anxiety, depression, and stress can have on SRNAs. After the approval from the Institutional Review Board, a survey was created on Qualtrics and was sent out through email as a solicitation to four CRNA programs: three programs being frontloaded curriculum structures and one school an integrated program. Responses to the survey included a total of 114 SRNAs at various levels of their education in CRNA schools. Results of the survey showed that SRNAs experience higher than normal, in some cases, extreme levels of stress, anxiety, and depression during CRNA school. Education on stress and positive coping should begin as early as the interview process for future SRNA candidates and continue throughout their career in anesthesia school. Furthermore, most students participating in the survey did not feel that their school or faculty supported their mental health and well-being. The conclusion is that the development of wellness programs and mental health education is necessary for SRNAs during CRNA school. These programs should begin early to help the SRNA develop positive coping mechanisms to utilize during CRNA school. More research is needed to determine if the structure of the CRNA curriculum frontloaded verse integrated influences the stress load that an SRNA experiences during CRNA school.

Occupational Hazards: Considerations for the Pregnant Anesthesia Provider

<b>Background</b>: Anesthesia providers are subjected to chemical, biological, physical, ergonomic, and psychosocial risk factors that threaten the individual's health. While pregnancy alone is not considered an independent risk factor for healthcare-associated occupational hazards, the fetus is rapidly developing and thus carries a much lower threshold to hazardous exposure than the adult.
<b>Purpose</b>: To evaluate participants’ confidence levels related to their knowledge of environmental considerations for the pregnant anesthetist before and after implementing a self-paced online course presenting the findings from a review of the literature.
<b>Methods</b>: Voluntary participants from a Midwest DNP nurse anesthesia program were invited to partake in a self-paced online course which included a pre- and post-assessment to evaluate confidence level related to the subject matter and its application into practice. Conclusion: Confidence levels in the participants' knowledge of the environmental risks for anesthesia personnel and pregnant anesthesia providers increased by 36.4% following implementation of literature review findings summarized into self-paced online course.

Ondansetron before spinal anesthesia in cesarean sections: An evidence-based educational intervention

Background and Review of Literature: Spinal anesthesia is the current gold standard of practice for providing care in elective cesarean sections. However, spinal anesthesia is commonly associated with untoward side effects including activating the Bezold-Jarisch reflex leading to significant hypotension and bradycardia. Current evidence-based literature shows the administration of a 5-HT3 antagonist, such as intravenous Zofran, can reduce these side effects as well as reduce the amount of vasopressors used to provide optimal patient care.
Purpose: This study explored the efficacy of an educational intervention on increasing the willingness and knowledge to utilize a 5-HT3 antagonist before a spinal anesthetic in cesarean sections
Methods: An email list of 25 active anesthesia providers at a large metropolitan hospital was acquired and presented the educational opportunity via email. This included a Qualtrics pre-educational questionnaire, an attached educational PowerPoint, and a Qualtrics post-educational questionnaire. These helped to determine current anesthetic practices, basic knowledge of Zofran, and willingness to adopt a 5-HT3 antagonist into anesthetic practices before spinal anesthesia.
Implications/Conclusion: The implementation of an educational intervention on eight anesthesia providers significantly increased education and knowledge. Alternatively, the educational intervention did not significantly increase willingness to change anesthetic practices due to the high extremely willing response rate in the pre-educational questionnaire.
Ondansetron Prior to Subarachnoid Anesthesia in Parturient Patients Undergoing Cesarean Section to Decrease Hypotension: A Practice Change Guideline

Background: Cesarean sections account for 31% of all births annually, with spinal anesthesia being the preferred method of anesthesia. The side effects from spinal anesthesia in the parturient include hypotension, bradycardia, and vasodilation. The medication ondansetron, when administered before spinal anesthesia, can help mitigate some side effects of spinal anesthesia.
Purpose: This DNP project was developed to educate anesthesia providers on the benefits of administering ondansetron before spinal anesthesia in a parturient scheduled for a cesarean section.
Methods: This DNP project collected quantitative and qualitative data through electronic pre-tests and post-tests. An evidence-based educational intervention was conducted with a retrospective chart review.
Implementation: Five anesthesia providers at a rural hospital in northern Indiana participated in this project. The providers were given a pre-test and then, one-on-one, provided with education from a PowerPoint presentation. Following the presentation, a post-test was administered. Additionally, a retrospective chart review was conducted three months before and three months after the educational intervention to determine if a practice change had been made regarding the timing of ondansetron administration.
Conclusion: The introduction of an educational intervention improved anesthesia providers' knowledge on the benefits of ondansetron before spinal anesthesia in parturients, with a statistically significant (p < 0.001) increase in pre-test to post-test scores. Additionally, a retrospective chart review indicated that before the educational intervention, ondansetron was given 0% of the time before spinal anesthesia in parturients, and following the educational intervention, it was increased to 50% of the time before spinal anesthesia
